LIABILITY & WORKER'S COMPENSATION
Our practice requires written or verbal authorization for you to be seen PRIOR to your worker's compensation visit. Date and time of injury, employer's name and address, worker's compensation carrier's name and address and a claim number must be given before or on the day of the visit. We will be happy to cooperate with requests for any additional information your carrier may need to process the claim. We require payment at the time of service for all liability claims. We will provide all necessary information for the patient to submit.

FEES
Just as we are committed to providing you with the highest quality orthopaedic care, we are also concerned with keeping our services affordable. An important way we accomplish this goal is by eliminating the need for billing. We request payment on the day of your visit. Prompt payment allows us to reduce our overhead without diminishing the quality of our service to you. Fees for initial patient visits are usually more than for a follow-up visit. We accept all major credit cards and ATM cards. Our billing office can provide you with a receipt for submission to your insurance carrier(s) for reimbursement. As a participating provider, the law requires that we submit all Medicare claims to Medicare directly to you. At the present time, Medicare pays 80% of the approved fee. By law, the remaining 20% is the patient's responsibility in addition to their annual deductible which we request at the time of service. We shall be happy to discuss your insurance questions or payments at any time. Please call our billing office at 973-538-0329.
