To prepare for your up-coming visit with us, please adhere to the following:

1. Bring your insurance card with you to your visit.
2. Check with your insurance company to see if they require you to obtain a referral for your visit. If you need one, speak to your primary care doctor in advance of your visit AND have the referral with you for your visit. Make sure your referal includes the exam and treatment.
3. If you have already been to the Emergency Room (or an outside facility) for X-rays, bring the actual films with you.
4. If you have had an MRI or other imaging relevant to your visit, bring the films with you.
5. Bring a current list of your medications with you to the visit.
6. Please wear clothing that will allow adequate exposure of the body part for which you are being seen. (For example: shorts for a knee, tank top for a shoulder) Gowns will also be provided.

Policy for new patients or prior patients with new insurance coverage who do not present an insurance card at time of service: 

If an insurance card is NOT PROVIDED, there are two options:

1.     Pay upfront in full for services rendered.  (Exact pricing is determined at the end of the visit.  Typically $200-$400). 

2.     Reschedule the visit.
