Thank you for choosing Tri-County Orthopedics for your orthopedic care. For your convenience, you may download, print, and complete our required patient forms below. Please remember to bring your completed forms, along with the following pieces of information, to your scheduled appointment:
- Co-payments (if required by your insurance)
- Insurance card(s)
- Photo ID
- Referrals (if required by your insurance)
All forms should be completed prior to your appointment.
Patient Instructions for Medical Record Requests
Tri-County Orthopedics has partnered with HealthMark Group to allow us to process requests more efficiently and deliver records to you as quickly as possible.
To submit your request for medical records, please use the button below and follow the instructions.
After entering your email, you’ll receive a secure link from HealthMark to access your request, no username or password needed. Follow the steps to submit your request, and you’ll be notified by email when your records are ready.
Do I need to create an account?
No. The system uses email verification and secure links, so there’s no need to remember additional login information.
How long does it take?
Most requests are completed within one to two business days, with many processed even sooner.
